Job Description

St Helens Cares is the new approach to delivering Health and Social Care in St Helens; bringing together a wide range of public and community service organisations to work in partnership, with a united goal of improving people’s lives and improving the place as a whole. St Helens Council is proud to be part of this partnership.

Key to this vision is Contact Cares; which aims to simplify the way we access health and social care services from nursing assessment to social care; reablement to audiology, safeguarding to occupational therapy (medicines management).

We are seeking to employ Intermediate Care Support Workers who enjoy working with adults aged 18+. The aim of the service is to help people with disabilities be as independent as possible in their own homes. You must be flexible and able to use initiative when putting into practice rehabilitation/care support programmes set up by professional staff/team leaders.

The successful candidate will need to demonstrate their ability to carry out short-term work with service users in the community, prevent hospital admission/facilitate timely hospital discharge or when required provide support in Brookfield Support Centre ( 24 hour Intermediate Care) residential setting.

If you feel you can work both as part of a multi-disciplinary team and on your own, this post will suit you. You should also be willing to learn new skills and participate in identified training programmes to enable you to work in new ways.
