Making A Difference as A Senior Social Worker in Continuing Healthcare

An exciting opportunity has arisen for an experienced Social Worker to take on a key role within St Helens Borough Council working as a Social Worker within the Continuing Healthcare Team. The team are responsible for assessment, review and care management of individuals with a range of complex needs.

Under the guidance and support of the Team Manager/Assistant Team Manager, the post will involve assessing individuals for NHS Continuing Healthcare in line with the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care. The Social Worker will promote best practice and provide professional advice to Nursing Homes and other providers, liaising with colleagues from other disciplines both internally and externally to the LA/CCG.

The post also includes case management, review and monitoring of cases and to provide support on a duty rota. The post holder will also be responsible for updating and amending care plans as required and will need experience in working on an electronic record management system.

We are looking for an individual who is passionate about the delivery of quality care for the residents of St Helens. You will possess excellent communication skills and be able to build effective internal and external relationships.

*A recruitment and retention allowance of £1500 per annum will be paid to Senior Social Workers. Please note this payment is due for review on 31/03/2025.
