Making A Difference as s Social Worker in Contact Cares
An opportunity has arisen for a driven and innovative qualified Social Worker to join The Integrated Health and Social Care Department within the Contact Cares Team. This large and busy multi-disciplinary team is the single point of access for health and social care referrals. We are looking to recruit motivated social workers to join our team of both qualified and unqualified Social Care staff, Nursing and Therapy.

We provide community and intermediate care and support services, as well as a 2-hour response to avoid hospital admission, under Crisis Response Criteria. This post will primarily be to facilitate discharges from Newton intermediate care hospital; however, the successful candidate will be expected to support the wider team when required.

The Contact Cares team also provide initial contact, assessment, care planning, onward referrals, and signposting services for a wide range of adult clients across health and social care. With a focus that stretches from prevention and early intervention, to reducing hospital admissions and readmissions the work is varied and interesting. This post will be Monday to Friday 8.45 -5.15. The team operate from 8.00 am until 10.00 pm 7 days per week to include Bank Holidays.
