Making A Difference as a Social Worker in the Integrated Discharge Team
We are looking for a Social Worker to join our Integrated Discharge Team based at Whiston Hospital. The hospital has recently been rated as “Outstanding” by CQC.
The successful applicant will be primarily based at Seddon Suite, at St Helens Hospital, which is a rehabilitation unit. It is part of the Cheshire and Merseyside Rehabilitation Network, which provides specialist assessment, treatment and rehabilitation for adults following traumatic injury or serious illness. You will also need to liaise with other Local Authorities and ensure good communication to help facilitate out of area discharges. There will also be some days working from home on a rota basis.
The Integrated Discharge Team comprises of a range of disciplines and completes assessments for St Helens MBC. We commission a range of services to ensure service users are discharged safely, but also maximise their independence by taking a strength-based approach to our assessments. As part of the Contact Cares range of services you will be required to be flexible and recognise the benefits of working collaboratively with colleagues across St Helens Cares.
