An excellent opportunity has arisen to join our team at Brookfield Support Centre for Older People.
The team is multi-disciplinary and works with Adults to promote their independence and prevent unnecessary hospital admission and facilitate timely hospital discharge.
The majority of the workload will be carried out using electronic systems and excel sheets e.g. Integrated Adults System (IAS) and lotus notes. You will also be expected to have excellent verbal communication/written skills as some of your work will be taking telephone calls/messages from service users, carers, the general public and other professionals.
These roles will be worked on a rota basis over 7 days per week.
