We are looking for a Social Worker/Senior Social Worker to join our Integrated Discharge Team based at Whiston Hospital. The hospital has recently been rated as “Outstanding” by CQC. Being based on site ensures that you are involved in discharge planning from a service user's admission to the ward. It gives you the opportunity to be fully involved in the multi-disciplinary team and decision making process.
The team comprises of a range of disciplines and also completes assessments for St Helens Council. We commission a range of services to service users are discharged safely but also maximise their independence by taking a strength based approach to our assessments. As part of the Contact Cares range of services you will be required to be flexible and recognise the benefits of working collaboratively with colleagues across St Helens Cares.
The pace of work within the hospital team is fast, but we still ensure peoples safety prior to discharge. St Helens is one of the best performing authorities in the North West in regard to low numbers of Delayed Transfer of Care and low number of admissions to 24 hr care.
