We are looking for an Older Persons Commissioning and Transformational Lead who will work across Health and Social Care in St Helens. This is an exciting role as is involves working across organisational boundaries and seeks to bring about deliverable improvements though multiagency working.
This is a new post and will be responsible for the coordination of Older Peoples National and Local agenda across Health and Social Care. Key activities include updating and coordinating the Older Persons Strategy and action plan, and a lead role on dementia, frailly and falls.
The post is situated within the Public Health Team. You will be required to liaise with all local agencies including the Acute Trust and the Mental Health Trust, Primary Care and third sector. You will be expected to work with the Council’s Public Health leads on the prevention strategy.
St Helens already operate an Integrated Care System that works very efficiently and collaboratively. You will be expected to ensure that ethos is maintained and therefore working with commissioners of services for older people.
In St Helens we operate agile working so the post will be a mix of home and office based working, the main base being Atlas House.
Must be registered with a professional body.
