Making A Difference as a Care Arranger in Market Relationships Team
The Market Relationship Team are looking for a temporary Care Arranger. This role is pivotal to aid timely discharges for service users from hospital and for those service users who are moving to or returning to a care home or are requiring a package of domiciliary care within their own home. The role involves establishing good working relationships with the care homes, domiciliary care agencies and a range of professionals to ensure a smooth transition for the service user and to support the hospital through demanding pressures.
The role is a temporary role until 31st March 2025.
