With a range of community therapy, community nursing, frailty, housing and social care services, our experienced multi-disciplinary community based team work closely with our clients, to promote independent living and support and plan their return back into the community.

If you are caring, responsible and good with people, we have a number of opportunities for you to join our team as an Intermediate Care Support Worker, to promote and support the rehabilitation provision from Brookfield Support Centre.
