We are looking for a Social Worker to join our Integrated Discharge Team based at Whiston Hospital. The hospital has recently been rated as “Outstanding” by CQC.
The successful applicant will be office based at Nightingale House – Whiston Hospital, with some days working from home on a rota basis. The hours will be 37 hours per week, which will be worked over 7 days on a rota basis, which will include one weekend a month.
The Integrated Discharge Team comprises of a range of disciplines and we commission a range of services to ensure service users are discharged safely, but also maximise their independence by taking a strength based approach to our assessments. As part of the Contact Cares range of services, you will be required to be flexible and recognise the benefits of working collaboratively with colleagues across St Helens Cares.
