Job Description
St Helens Borough Council, Torus and St Helens CCG are working in partnership to expand our short term transitional accommodation provision at Brookfield Centre, for vulnerable adults who are in need of short term reablement support or at risk of hospital admission, due to health or care crisis.

With a range of community therapy, community nursing, frailty, housing and social care services, our experienced multi-disciplinary community based team work closely with our clients to promote independent living and support and plan their return back into the community.

If you are caring, responsible and good with people, we have a number of opportunities for you to join our team as an Intermediate Care Support Worker, to promote and support the rehabilitation provision from Brookfield Support Centre.

You will work with our clients to implement rehabilitation and care programmes, which have been developed by our multi-disciplinary team to encourage independence and self-management.

You will have experience of caring for older people and be able to provide support with personal care, emotional and social needs in a kind, caring, trustworthy and respectful manner that preserves dignity and self-respect at all times.

You will have some knowledge of client handling and safeguarding of vulnerable adults, though training will be given, and be able to clearly and confidently communicate with clients.

This role will require you to have a flexible approach to covering shifts including evenings, weekends and bank holidays for which an additional allowance is payable. Your hours of duty will be between 7.30 am and 10.00 pm.
